East and North Hertfordshire Clinical Commissioning Group will be showcasing the work of their Care Home Vanguard, which will allow delegates to learn from their successes, benefit from lessons learned and take away practical ideas to integrate Vanguard best practice into improvement programmes.

The format of the showcase is:
• A national overview on Vanguard learning, from William Roberts, National Lead, Enhanced Health in Care Homes, New Care Models Programme

Four themed workshops:
1. Improving hospital discharge processes – the role of the ‘Impartial Assessor’ and the care home ‘Red Bag’ scheme
2. Multi-disciplinary working – Medicine Optimisation in care homes and the East of England Ambulance Service Trust’s Early Intervention Vehicle
3. Technology in care homes – NHS.net and real-time GP access to patient records
4. Workforce development – up-skilling care home staff to prevent avoidable hospital admissions
